Contractor Approval & Agreement

Prior to any purchases of FiberTite roofing membrane, you must complete our Contractor Approval and Agreement Process. To expedite the process, we are now making this process available to you online through the FiberTite e-Contractor Approval & Agreement System.

Order Form

Use our convenient online order form to place your orders. Either fill it out online, print, and fax it, or print the blank form and fill it out by hand. Fax your order forms to Customer Service at 800-649-2737.

Credit Application

Before your material can be produced and shipped, it is necessary to completely fill out our online credit application. Once you have correctly completed the form, we can begin the process of adding you to our team of successful suppliers as soon as possible.

Pre-Installation Notice

The FiberTite Pre-Installation Notice must be executed by an officer of the Authorized Roofing Contractor and submitted to Seaman Corporation before material shipments can be released. Click here to fill out our convenient online Pre-Installation Notice and submit directly to Seaman Corporation.

Service Request Form

The Service Request Form allows "you" to request service directly from Seaman Corporation or our representatives on a warranted FiberTite Roof System at any time day or night. Seaman Corporation warranty service administrator will use e-mail to keep you updated. The accuracy, integrity and fullness of the information will have a direct impact on the timeliness of as well as our ability to provide the requested service(s).
